Home-based care presents unique safety challenges, as clinicians work independently in unfamiliar and unpredictable environments. This session will examine how structured safety programs, ongoing education, clear policies, and a strong safety culture can help address risks related to the home environment, patient and household behaviors, infection exposure, travel, weather, and personal security.
Using Croí Health’s safety program as a practical model, the presentation will explore clinician orientation and onboarding, recurring education, staff in-services, safety communications, incident and near-miss reporting, and the role of an interdisciplinary Safety Committee. Attendees will learn how organizations can identify safety concerns, recognize trends, implement targeted interventions, and continuously improve practices to protect clinicians and patients.

By the end of this session, participants will be able to:
- Identify common safety risks associated with providing care in patients’ homes.
- Describe key components of an effective home-based clinician safety education and training program.
- Recognize how incident reports, near misses, employee concerns, and safety trends can inform organizational improvements.
- Explain the role of an interdisciplinary Safety Committee in addressing workplace safety concerns.
- Apply practical strategies to strengthen clinician preparedness, reporting, communication, and safety culture in home-based care.
