Caregiving 

FREE Initial Assessment: Our Registered Nurses lead the clinical side of our company and visit each client at their place of residence to evaluate the type of care needed, discuss options and develop a Plan of Care at no cost.

Benefit Determination: We counsel clients on the most advantageous ways to activate Long Term Care insurance, provide $2,500/year to give family caregivers a break and help families decide on ongoing Home Care.

Developing an Ongoing Plan of Care: We incorporate all the orders and recommendations from a client’s physicians, case managers, healthcare providers and our own initial assessment in order to develop an ongoing Plan of Care. This plan will describe all the services required, the proposed frequency, cost of each service, potential equipment and supplies needed, and possible payment sources.  

We provide:

Personal Care

• Home safety evaluations, fall prevention plans and recommendation for assistive devices

• Assistance with daily activities such as mobility, toileting, bathing, dressing and transfers

• Medication Management, Light Housekeeping, Errands, Meal Preparation and Companionship

• Transportation and accompaniment to healthcare appointments, picking up medications, picking up groceries 

and running errands

Dependable Transitional Care from Hospital or Rehab to Home

Our RN works with the Discharge Team to build the Plan of In-Home Care; we pickup medications; run errands; light housekeeping; prepare meals; immediately implement the of Plan of Care; flexible scheduling: hourly, 24/7 or Live-in care.

Specialty Care and Continuity of Care

• We Manage Chronic Conditions: Parkinson's, Alzheimer’s, orthopedic, cardiac, pulmonary, neurological, 

diabetes and other conditions

• Post-surgical care and in-home assistance after cancer treatment or other inpatient care

• Wound management, injections, catheter and other care under the supervision of our agency’s RN

• Personal Care assistance for in-home hospice care
